The provincially approved Adult Respiratory Therapy (RT) Ventilator Flowsheet has been developed in collaboration with the RT departments across the province. This form was approved for RT use by the following SDOs – Winnipeg Regional Health Authority, Shared Health HSC, Prairie Mountain Health, and Interlake Eastern Regional Health Authority. Final approval was received through the Provincial Health Information Management (PHIM).

The intent of the form is to:

• Strengthening RT continuity of care and handoff accuracy when patients are transferred between sites
• Ensuring all sites are following standards of practice in ventilator charting
• Ensuring thorough and standardized respiratory assessment is performed with each ventilated patient
• Reducing variability in care practice across sites
• Streamlining orientation of new RT staff and students
• Reducing errors compared with multiple documentation methods
• Enhancing data collection for QI and research
• Allowing for easier integration into electronic health records

Please share this information with the appropriate contacts within your organization as needed. Any questions should be directed to your site RT clinical leads or manager, who will oversee the implementation of this form. The rollout is a site-level responsibility and should be completed when operationally feasible. Targeted implementation is May 1, 2026.
