Staff Safety Event Report
This form is used to document all Staff Safety Event reports within Southern Health Santé Sud.
A Staff Safety Event report needs to be filled out for all staff injuries or events that could have resulted in injury – these events are called near misses.

Why do I need to report an Injury / Near Miss?
- Injury and near miss reports helps identify potential hazards and prevent future incidents, leading to a safer workplace for everyone.
- Assistance related to your injury can only be given once an SSER has been filled out. This assistance can look like an Ability Management Officer helping you through the Return to Work process, an Occupational Safety & Health Officer assisting your manager in investigating the cause of your incident, or a Labor Relations Officer assessing the situation in cases of extreme violence.
What happens after I send in an SSER?
Your report will be reviewed, and you may be contacted for further information. Further follow up and assistance may be provided as necessary.
Do I need to complete any other reports?
If the incident involved potential harm to a patient or damage to equipment/property, you must complete a separate patient safety report in the RL system.
