The questions from the initial nutrition screen are included on the CLI.4510.PR.002.FORM.24 Adult Nursing Admission History and the CLI.6110.FORM.008 Dietitian Referral Form-Inpatient have replaced the initial nutrition screen.

Please remove all copies from the unit and discard.
