We are offering employees an opportunity to voluntarily self-identify, whether you have not had the chance to do so in the past or would like to update your information. Building a workforce that reflects the diversity of the communities we serve is essential to delivering culturally safe, responsive, and equitable care.
The information gathered through self-identification is not about “checking a box.” It helps us better understand our workforce and identify where action is needed—informing policies, programs, and supports that create a more inclusive and representative organization.
To support this work, we invite you to confidentially share information about your ethnicity, gender identity, disability status, and other aspects of your identity. This information will help ensure our workplaces and services are supportive, safe, and inclusive for everyone.
Participation is voluntary and takes only a few minutes. You may choose to answer some, all, or none of the questions, and each includes an option to select “I wish not to declare.”
All responses will remain confidential, and your participation is fully optional. Information is stored in your employee electronic record; however, individual responses are not shared. Summary information is used to support workforce planning, equity initiatives, and organizational improvement.
