Over the fall, we made significant progress in revisioning and realigning the Primary Care Program within Southern Health-Santé Sud (SH-SS).

In recent months, all remaining SH-SS clinics previously managed by facility leadership have transitioned under the primary care reporting structure. This alignment under a unified leadership model is an important step forward. It will promote integrated communication, enable us to share expertise and help achieve regional consistency and standardization – all with the goal of growing and strengthening the Primary Care Program across SH-SS.

The primary care leadership team has worked diligently to bring this vision to life, and their collaboration and expertise have been instrumental in making these changes happen. We also want to extend our sincere appreciation to clinic staff and providers for embracing this transition and working together to ensure a smooth and positive change.
