Student Checklists
Select the checklist appropriate to your upcoming placement, complete it in full, and submit it to the receiving coordinator two weeks prior to your practicum start:

Student Checklist
- Empathy Video (3:49 minutes)
- PHIA Video (24:07 minutes)
- SH-SS Declaration of Confidentiality
- Will require signatures from student and a witness (can either be the instructor or me).
- PHIA for Health Care – Manual Pledge

- Protecting and securing confidential information is everyone’s responsibility
- PHIA
- Learning Management System (LMS) PHIA – Instructions to complete online PHIA training using the casual employee/students LMS account (no tracking, requires manual pledge). Turn off your pop up blockers.
- Login: username: casual.employee password: password123
- Select Learning History and click Launch next to the PHIA for Health Care – Manual Pledge. Once completed, print a copy of your screen or take a picture to send to the receiving coordinators with your signed Declaration of Confidentiality form
- Note: It is recommended that the course be completed in one sitting, as it will always start again from the beginning when launched from the History tab.
