Provincial Clinical Guideline – Advanced Glucose Monitors in Home Care
The provincially approved Home Care: Advanced Glucose Monitors in the Home Care Setting Clinical Guideline aims to enhance and standardize the use of advanced glucose monitors in home care. It provides comprehensive support for eligible clients who require assistance with these devices, ensuring the delivery of consistent, high-quality care. This guideline has been developed in collaboration with representatives from the Home Care programs across all five Health Regions and will be effective as of August 11, 2025.
To support the successful implementation of this guideline, an education package has been created. This resource is available to all Health Regions to facilitate training and promotes a comprehensive understanding and application of Advance Glucose Monitors in Home Care.
