Staff Safety Event Reporting
With the upcoming rollout of the new Radical Logic (RL) Patient Safety Reporting Program in October, we’ve received several inquiries regarding how to report staff injuries.
STAFF INJURY REPORTING WILL REMAIN THE SAME
Sites may continue using pg. 4 of the old form until existing copies are depleted. Page 4 of the previous Safety Event Report has been revised and renamed to the Staff Safety Event Report (SSER) form and Poster – linked for distribution.
It is important to note to staff that submitting an incident to RL (Radical Logic) does not automatically notify the Workplace Safety and Health Team. As a result, processes such as the WCB claim, incident investigation, or any other support you may expect following a workplace injury will not begin based solely on an RL submission. To initiate these steps, an SSER must be submitted to the manager / direct supervisor. This ensures the appropriate teams are informed and the necessary actions can be taken.

An online SSER form will be made available on StaffNet in October. Communication will be shared once the online form is available.
FAQ:
IF I SUBMIT AN RL, WILL AN SER BE FILLED OUT WITH THE INFORMATION I PROVIDE IN THE RL?
No. An SSER must be completed for all employee injuries.
For example, if both you and a patient are injured in a fall, you are required to submit to RL (Radical Logic) for the patient and an SSER for your own injury. This ensures the appropriate follow-up and support processes are initiated.
IF I SUBMIT AN RL, WILL WCB KNOW ABOUT IT?
No. The WCB Process can only begin when you submit an SSER.
CAN I SUBMIT AN RL TO ANOTHER SDO PORTAL?
No. RLs submitted through other SDO portals do not reach the appropriate channels within SH-SS. To ensure your report is received and properly addressed, please submit RLs through the designated SH-SS portal only.
