Project Status: Complete

The Palliative Care team identified their referral process had potential to be improved through LEAN principles. Our goal for this project was to review and measure the challenges currently faced in the referral process and implement strategies to achieve a better response time between receipt of referral to first initial contact with client/family. The team also wanted to reach a more consistent approach to assigning referrals to ensure team members were not duplicating tasks or doing more work than necessary.
