Care Plan Standardization

Project Status: Completed

In November 2017 a Yellow Belt Lean Project was initiated to focus on improving Care Plans for Home Care Clients. Our team is focusing on making sure Care Plans have accurate information, clear plain language, standardized format and length, and ensuring these changes are maintained. We also hope it will speed up the process of implementing care into the client’s home. Standardizing the form and process is our goal and using our measurements collected by the Case Coordinators, Resource Coordinators and the Home Care Attendants, we are confident that we can provide concise accurate care with less error and missed care occurring.
